You cart vital signs within a hospital usually every four hours, unless there have been a tweaking in a lenient ( level of conciousness, topple, something" not quite right") In areas such as ICU or CCU, ER you may be required to do them more recurrently. Anytime you are ask to do them. I have never hear of a contraindication of vital signs unless it is comfort charge and the family requests they not be done, or the long-suffering is deceased.
Indications for taking central signs : they were ordered by a physician or by hospital protocol.
Contraindications: you don't want to see how desperate things really are!
One of the contraindications to taking vitals might be an upset, screaming kid. If you take vitals while a babe-in-arms is screaming, you can get a respiratory rate of 70 and a heart rate of 200, but those aren't their true central signs.
Similarly, if someone is agitated and throwing things at the nurses, it might not be a great time to take vitals.
haha.. darz.. contraindications nlng kuwang..
